To complete the enrollment, we must also receive two additional forms.  These forms can be downloaded from the links below.  Please print them out, sign them, and deliver, e-mail, or snail mail them to Victory Baptist Church.  Each participant must have the signed consent forms completed prior to participating in VBS.
